From constraint to handover

  1. Challenge

A clinic group received referral packets as scans. Staff retyped the same fields into the record, and missed pages were found late.

  2. Approach · 01

Referral packets arrived as scans in a shared inbox. Staff retyped the same fields into the record, and a missed page was discovered late, usually by the person who needed it. The fitting engagement was document processing, not a new clinical system.

  3. Approach · 02

The build lifts the agreed fields and parks low-confidence pages in a review queue. A low-confidence read is not filed as if it were certain. The record system the clinic already uses stays the source of truth. We connect to it. We do not replace it.

  4. Approach · 03

After go-live, the small breaks do not belong in this block. They belong in a support month, which this story deliberately does not include. Mixing them would have hidden a new queue inside a finished project.

  5. Result

Document processing that lifts the agreed fields and parks low-confidence pages in a review queue instead of the general inbox.

A composite is useful when it shows the boundary. It is not a logo and it is not a promise that your version will match.

The hourly figure is the same starting rate as the service the story belongs to.
